Convicted Person Registration Form (Felon Registration)
A convicted person with 2 or more felony convictions or 1 class A felony must complete a registration in person within 48 hours of establishing residency, employment, becoming a student, entering Douglas County 5 or occasions in a 30 day period or staying longer than 48 hours in Douglas County. The application must be submitted in person at 1038 Buckeye Rd, Minden, NV. For questions call 775-782-9933

A convicted person must complete as needed when a registrants address, employment, license, school, or vehicle(s) change. Must be completed in person and have a photo ID.
